    • Training in the military
  • Must have completed at least seven hours of AIDS training
  • Provide a list of all states where the applicant has applied for a license in the past
  • Must provide a statement about and explain any issues:
    • Their overall physical health
    • Their overall mental health
    • Substance abuse issues
    • Felony convictions
    • Past disciplinary actions, including the revocation of a license in any state

 

Hopefully, when you write your statement, you’ll be reporting negatively to all of these, but if not, you need to include a detailed explanation of why they apply to you.
